ما هي المياه الزرقاء (الجلوكوما) ؟ 

الجلوكوما هي ارتفاع ضغط السائل المائي داخل العين مما يؤدى إلى إتلاف تدريجي بالعصب البصرى. ويحدث ذلك نتيجة خلل في النسيج المسئول عن ترشيح السائل المائي إلى خارج العين. ويؤدى ارتفاع ضغط العين إلى موت الخلايا العصبية التي تكون العصب البصرى.

 لماذا سميت الجلوكوما بالمياه الزرقاء؟

 سميت الجلوكوما بالمياه الزرقاء لأن في بعض حالات الجلوكوما الحادة أو الخلقية في الأطفال يكون لون الحدقة مائل إلى الزرقة. ولا يحدث هذا في الجلوكوما المزمنة. 

ما هي أعراض هذا المرض؟

 خطورة هذا المرض تتمثل في انعدام الأعراض، حيث (لا) يشعر المريض بصداع أو ضعف الرؤية أو أي أعراض أخرى وبالتالي لا يذهب المريض إلى الطبيب. وعادةً ما يتأخر المريض حتى يتآكل العصب البصرى في كلتا العينين و يسبب انكماش تدريجي في المجال البصرى دون أن يشعر المريض حتى يصاب بالعمى. وذلك على العكس من الاعتقاد السائد بأن الجلوكوما يصاحبها صداع شديد واحمرار بالعين. 

كيف يمكن تشخيص هذا المرض؟ 

عن طريق الكشف الدوري وأيضاً الفحص الدقيق للعصب البصرى أثناء فحص قاع العين. وفى بعض الحالات نلجأ إلى فحص مجال الإبصار بأجهزة متقدمة جداً كي نسجل وظيفة العصب البصرى .

 هل العلاج يستمر لفترة محدودة أم مدى الحياة؟

 العلاج الدوائي يستمر مدى الحياة مع ضرورة المتابعة الدورية. 

ما هو الجديد في تشخيص الجلوكوما؟

 توجد الآن أجهزة حديثة لتصوير العصب البصرى بالليزر. وهذا الفحص مهم للتأكد من التشخيص وتحديد مدى التلف الحادث بالعصب البصرى، ومن ثم تحديد العلاج المناسب. 

فحص مجال الأبصار ( الميدان البصري): 

هذا الفحص يهدف إلى معرفة حساسية الشبكية والعصب البصرى فى كل نقطة من نقاط المجال البصرى ( الحيز الذي يراه الإنسان حول نقطة تركيز البصر) ويتم هذا الفحص بواسطة أجهزة حديثة تعمل بالكومبيوتر . 

التصوير المقطعي لأعصاب الشبكية بالليزر (OCT):
حديثاً يمكن تصوير العصب البصرى بالليزر لقياس سمك الأعصاب الحسية التي تغذى العصب البصرى ، وبالتالي مدى تأثرها بارتفاع ضغط العين.

وكيف يمكن علاج هذا المرض؟

هذا المرض يمكن علاجه بالقطرات التي تخفض ضغط العين أو بالليزر. ولكن بعض الحالات تحتاج إلى التدخل الجراحي.
